Health and safety reports

Health and safety reports are to be prepared by the service provider (SP) and submitted to Oxfordshire County Council Corporate Landlord (OCCCL) on a monthly/quarterly basis as required within the contract. Reports should include details of the following:

Reportable accidents / dangerous occurrences

  • All reportable accidents or dangerous occurrences involving SP staff or contractors while visiting or working on Oxfordshire County Council sites.
  • Any dangerous occurrences or incidents involving SP staff or contractors affecting any other persons on the site.
  • Any known near misses involving SP staff or contractors that could have had consequences.

Visits or involvement of enforcing authorities

  • Any visits or involvement of the Health and Safety Executive, local authority, Environment Agency, Fire and Rescue Service etc. concerning work being managed or overseen by the SP.
  • Any improvement or prohibition notices and pending prosecutions issued by any of the enforcing authorities concerning work being managed or overseen by the SP.

Details of any issues in 1 and 2 must be reported by the SP to OCCCL as soon as possible after they occur.

Monitoring, audits or site inspections

  • A summary of the results of contractor performance monitoring.
  • The results of any contractor or site health and safety audits carried out.
  • The results of any site health and safety inspections.

Customer / client / other feedback

  • Any health and safety feedback from customers, clients or end users.
  • Any health and safety feedback from members, general public etc.

Contractor issues

  • Poor health and safety performance of current contractors.
  • Use of new contractors.
  • Use of Construction Skills Certification Scheme (CSCS) etc.
  • Any Construction (Design and Management) Regulation issues.

New initiatives, innovations or improvements

  • Any new initiatives that have been introduced to improve health and safety performance at all stages of the construction process.
  • Any innovations or improvements introduced to improve health and safety performance at all stages of the construction process.

Training

  • Any new health and safety training initiatives for SP staff.
  • Any new health and safety training initiatives for site users or others.
  • Circulation of new health and safety information.

General issues

  • Any health and safety issues that may have occurred on or involving projects at Oxfordshire County Council sites that are not adequately covered in the preceding sections.

Summary

  • An overview of all health and safety issues regarding work managed or organised by the SP for Oxfordshire County Council during the last month/quarter as required within the contract.
